Austria's mandatory health insurance system for employees is governed by the Austrian Health Insurance Act (Sozialversicherungsgesetz). All employees in Austria must be enrolled in statutory health insurance from their first day of employment, with no exceptions or opt-out provisions.
The health insurance scheme covers medical treatment, hospitalization, prescription medications, dental care, and rehabilitation services. Employees contribute a percentage of their gross salary to the insurance fund, currently set at approximately 7.65 percent, while employers contribute around 5.15 percent. These rates are subject to annual adjustment by the Austrian government based on healthcare costs and economic conditions.
The system is administered by the Austrian Health Insurance Fund (Österreichische Gesundheitskasse, ÖGK), which consolidated multiple regional insurance carriers in 2020. This consolidation streamlined administration and standardized benefits across all provinces.
Health insurance contributions are calculated on the employee's gross monthly salary up to a statutory ceiling. Self-employed individuals and certain other worker categories have different contribution structures but remain within the mandatory system.
Employers must register employees with the health insurance fund within specified timeframes and remit contributions alongside income tax and social security payments. Payroll teams must ensure accurate calculation of health insurance deductions and timely submission of contributions to avoid penalties. Employees receive a health insurance card (e-card) granting access to covered services throughout Austria and other European Economic Area countries under reciprocal agreements. Non-compliance with registration or contribution requirements can result in significant fines and back-payment obligations.
